Lakes Area Veterinary Hospital will not be responsible for damage or destruction to any items left with your pet. 

To ensure the protection of all the pets under our care, they must be up-to-date on vaccinations, free of intestinal parasites, free of fleas/ticks and dogs must be on a heartworm preventative. If any of these are needed, I give permission to do any necessary treatment my pet needs while in your care.

While I accept that all procedures will be performed to the best of the abilities of the staff at this hospital, I understand that no guarantee or warranty has been made regarding the results that may be achieved.
